What Is a Coronary Artery Calcium Score?

A coronary artery calcium (CAC) score is a number that reflects how much calcified plaque has built up in the arteries that supply blood to the heart. It is obtained with a quick, non-invasive CT scan of the heart. When cholesterol plaque forms in the artery walls over years, the body often deposits calcium as part of the healing process — so calcium is a visible "fingerprint" of underlying atherosclerosis (hardening of the arteries).

Think of it this way: the more calcified plaque present, the more atherosclerosis has accumulated, and the higher the long-term risk of a heart attack or other cardiovascular event.


How the Test Is Done

The CAC scan is one of the simplest heart tests available:

- It uses a non-contrast, ECG-gated CT scanner — no IV, no dye, no medications, and no fasting required.

- The scan itself takes only a few minutes, and the whole visit is typically under 15 minutes.

- Radiation exposure is low, roughly 1 millisievert — comparable to a mammogram and only a fraction of many other CT scans.

The result is reported as an Agatston score, which combines how much calcium is present and how dense it is across all the heart's arteries.


What the Numbers Mean

Scores are generally interpreted along these lines:

- 0: No detectable calcified plaque. This carries a very low (though not zero) risk of a cardiovascular event over the next several years — sometimes called the "power of zero."

- 1–99: Mild plaque burden.

- 100–299: Moderate plaque burden.

- 300 or higher: Extensive plaque and substantially elevated risk.

Higher scores mean higher risk in a step-wise fashion. A score above 300 in someone not previously known to have heart disease can carry a risk of major cardiac events similar to that of a person who already has established heart disease. Roughly one-third of people with scores above 1,000 have a cardiovascular event within three years.

Results are also often reported as a percentile compared with others of the same age, sex, and background, which helps put the number in context. At any given age, women tend to have lower scores than men.


Why the Score Is So Useful

The CAC score's main strength is that it measures disease that is actually present, rather than only estimating risk from factors like age, blood pressure, and cholesterol. This adds real value:

- It refines risk beyond the standard calculators. The score can move a person into a higher- or lower-risk category than their risk-factor profile alone would suggest, sharpening decisions about preventive treatment.

- It personalizes the decision about statins and other therapies. A higher score supports starting cholesterol-lowering medication (and aggressive lifestyle changes), while a score of zero may allow safely delaying or avoiding certain medications in appropriately selected people.

- It catches hidden risk. Some people with few traditional risk factors still have significant plaque — and this test can reveal it.

- It motivates action. Seeing an actual measure of one's own artery health often improves commitment to healthy habits and to taking prescribed medications.

Why It's Gaining Traction

Several developments have pushed CAC scoring further into everyday prevention:

- Strong long-term evidence. Decades of large studies confirm that CAC is one of the most powerful available predictors of future cardiovascular events, and it is now endorsed by major cardiology guidelines as a decision aid.

- Guideline support. Professional societies recommend considering CAC to help guide statin decisions in people at borderline or intermediate risk when the right course isn't clear.

- Opportunistic detection. Coronary calcium can increasingly be spotted on chest CT scans done for other reasons — with no extra scan, radiation, or cost — expanding who can benefit.

- Artificial intelligence. New AI tools can automatically detect and measure coronary calcium, making reporting faster and more consistent.


Who Should Consider It — and Important Limits

Despite growing enthusiasm, CAC is not currently recommended as a universal screening test for everyone. It is most helpful for adults (generally ages 40–75) at borderline or intermediate risk in whom the decision to start preventive medication is uncertain, and sometimes for younger lower-risk people with a strong family history of early heart disease. It is less useful under age 40, when calcified plaque is uncommon.

A few important cautions:

- A score of zero does not guarantee safety. It does not detect soft, non-calcified plaque, and it can be falsely reassuring in smokers, people with diabetes, or those with a strong family history.

- The test does not measure blockages. It reflects overall plaque burden, not whether an artery is narrowed enough to limit blood flow. It is not used to diagnose the cause of chest pain.

- It can lead to more testing. A positive result may prompt additional scans or evaluations, and incidental findings (such as lung nodules) sometimes turn up.


The Bottom Line

A coronary artery calcium score is a fast, low-radiation, and increasingly accessible way to look directly at the health of the heart's arteries. Used in the right person and interpreted alongside overall health, it can meaningfully sharpen decisions about how aggressively to prevent heart disease.
